题名: Metabonomics study on identifying insulin resistant individuals by UPLC-Q TOF MS

摘要: The insulin resistant state exerts only subtle abnormalities in asymptomatic individuals, which is in contrast to the type 2 diabetic state. Furthermore, although obesity is one important risk factor for the development of insulin resistance, not all obese individuals are insulin resistant and there are also a rare number of lean insulin resistant subjects. So, the identification of insulin resistant subjects by a straightforward laboratory test is urgently needed to establish a valid and sensitive analytical procedure for the detection of this subtle metabolic derangement. The present study was initiated to investigate the diagnostic power of analyzing the metabolic pattern in spot urine for the identification of insulin resistant individuals. The spot urine of 52 non-diabetic individuals was analyzed by a metabonomics approach using ultra performance liquid chromatography hyphenated to mass spectrometry. The bioinformatical evaluation of the resulting more than 19000 metabolite ion masses per individual resulted in four clearly separated groups, namely insulin sensitive lean and obese subjects and insulin resistant lean and obese subjects. The predictive power was assessed by internal and external validation. Among the identified metabolites differentiating the groups are 3-hydroxyhippuric acid, xanthine and phenylacetylglutamine. Our data provide for the first time evidence that the metabolic pattern in spot urine differs between insulin resistant and metabolically healthy individuals, offering prospects for a possible new screening strategy for insulin resistant individuals in future times.
